Having issues getting forms to work...

Hey Everyone, I need some help.

I built a form using Web Form Builder lite and tested it out on the S-Drive and it was very easy and straight forward to do. A default confirmation page was displayed and I was able to edit the format of the email notices and everything worked.

I've purchased Web Form Builder Full so I could export the same form and host it on my own server. I've installed the latest version of WAMP (2.5 I think) and have copied the exported form folder ("form") and php file ("form.php"). They are now on my WAMP server in D:\wamp\www\site1.

According to the export > manual server setup instructions I should copy the provide code and add it to a file (e.g. index.htm") that in the D:\wamp\www\site1 directory with the form.php file and the "from" directory. I've done that and when I open http://server/site1/index.htm I get a blank page. In d:\wamp\www\site1\form\storage\fb_error.log.php is recorded:
Wed, 07 Jan 2015 22:53:17 +0000: Failed to parse HTML form.
Wed, 07 Jan 2015 22:53:17 +0000: Couldn't open or read: D:/wamp/www/Site1/form.html. That file is actually located at D:\wamp\www\site1\form\form.html.

If I open http://server/site1/form/form.html in a broswer, I can see my form but two odd things happen. If their are validation errors i get something like this "Undefined index: y-key in D:\wamp\www\Site1\form\fbapp\inc\validator.inc.php" even though the y-key entry is not required in the form.

If I open http://server/site1/form/form.html in a broswer, and complete the form and submit, I get a blank page rather than the expected confirmation page.

And finally, the email notification that is generated when the form is submitted is formatted differently that when it's on the S-Drive.

Any thoughts on how i can proceed to make this work would be appreciated. Thank you.

Try removing the script wrapping the iframe code that the form builder provides.
I can't hear what I'm looking at.
It's easy to overlook something you're not looking for.

I use WAMP 2.5 32 bit version (64 bit is still not 100% ready for prime time) and forms will work fine with it.

Did you re-export the form using Export>Manual Server Setup?
Or are you using the one you made for S-Drive?
