Hello I am using version 2.3 of the form builder and it has been working just great for several years until a week or so ago when something changed. My hosting company has a history of changing permissions and settings without warning so I thought I would check and send you the errors I got and hopefully you would recognize and point me in the right direction for finding what is broken. I uploaded the servertest file to the domain but I can't tell which should be turned on or off. The domain is www.puppypatchlabradoodles.com

I have only seen one other forum post for a similar problem but I am not saving to a database so not the same. I am just sending the email
Do you recognize this error? Any help would be appreciated. Thanks
