I have a form up (http://glenedsoccer.com/volleyball/registration-2013/) and it works fine UNTIL i add 1 question that uses payment then i get the following error:

SQLSTATE[28000] [1045] Access denied for user 'glenedso_vb'@'127.0.0.1' (using password: YES)

if i take out that 1 question all is well so i know the mysql is correct

Hmm, looks like you might not have all files uploaded, or more likely you are missing a setting.
On the number field where you enter the amount of the payment, under properties did you select a payment type? Which type? Then on the settings page did you enable form payments then check the box 'Enabled' by the Payment Options, then enter your paypal details?

If all your setting look good I'd try and upload all the form files again. If that doesn't do it I'd open a support topic and see if a CoffeeCup guru has a better answer. Please post back here though and let us know what you find.

All files uploaded, checked. and reuploaded to make sure on a file by file basis

Radio buttons
options are
1 child:90.00
2 children:170.00
3 children:250.00

Required is checked
Payments - "Price Input" is selected

settings/Payments:
Enable form payments is checked
fixed amount is 0.00 - USD -Custom $ symbol is checked (tried both ways on this option)
Invoice lable has something there
payment type: Third party gateway is selected
payment options paypal website payments standard is chosen and Enabled is checked
account info is filled in

The data IS in the database just the payment doesn't work :(

If the data is going to the database, I'm lost. Probably something simple, usually is, but I'd open a support topic and see if someone at CoffeeCup can clue us in. Sorry.
