Form builder to Mysql - Post ID 231350

I have been trying to setup a web form. Each time I try to put in the data save (MySql), it brings me back to the settings page. (See attached). What seems to be the problem?

Did you enter your server mysql database info?
I can't hear what I'm looking at.
It's easy to overlook something you're not looking for.

It has been a long time since I used this product. I did the export, but it would go straight to my file on PC.
