The applicant is suddenly receiving the message "failed to send file" whenever someone fills out the form.

Ihave changed the SUBMITTED to: address and also used the cc:, but still nothing comes through.

I CAN see in the CSV log that the applicant sent the file, but I am not receiving a copy of the applicant's file.
