Not sure if I understand you correctly but could you just use a text element below or above each box? I have used WFB quite a bit but I am not sure if there is another work around.

Another thing you might want to try is filling out the form and taking a screen shot as an example. Then add an example section to your form showing how a perfect submission should look.
