Error in Form php file - Post ID 292181

All of a sudden I am getting an error when trying to access a contact form for a local non-profit Wordpress site. The contact form is located here: . When I go to that page I get the following error message:

"Fatal error: Uncaught Error: Class 'FormPage' not found in /home/rascal6/public_html/wp/suggestion-form/fbapp/php/ Stack trace: #0 /home/rascal6/public_html/wp/suggestion-form.php(6): require() #1 {main} thrown in /home/rascal6/public_html/wp/suggestion-form/fbapp/php/ on line 15"

I don't know much about this stuff. I did use Notepad ++ and opened the file referenced,, and here is line 15:

$myPage = FormPage::GetInstance( $myName );

As I say I am not familiar with the php stuff.

I opened the form in WFB, It previews fine. Saved it again and reuploaded with same results.

Is there anyone out there that can shed some light on why this error is all of a sudden happening?

Sorry for the post. I did it in panic mode. Turned out it was a problem with the PHP version.

I should have read the post referring to HP version 7.2.

I went back to 7.1 and all forms worked.
