and the remaining references to Coffeecup? This looks pretty bad on a customers reply form.

Anytime I try to edit the following or any portion of it, the form will not return any information at all.

That would mean you edited the page and included invalid HTML. You should try and validate the content you put in there before publishing your form.

I copy the code out of the Body section of the "Customize Notification Message" and paste it into the HTML editor. I clean up the code applying indents and line returns as needed. From there it's pretty easy to add/delete as you can see the results of your changes via the "Local Preview" menu option.
