I see on the forums this has been covered before and I apologize, but I just can not see where my error is.

Hosting on my own servers - so permissions, etc. should not be an issue.

When I past the code from the manual forms exporter - I end up with a blank form.

I would start with running this through the HTML and CSS validator as it's pretty messy. Try first also removing all the millions of scripts you have running and start there to see if it's the form that's not working or if your scripts are overlapping it as you have way to many running.

Might I also suggest you work on learning some CSS as much of your coding is not going to validate very well at all due to deprecated code practices.

Once you check all the validation out and correct all the problems and test the scripts removing let us know what you find out. Good luck on it
