Hi ... I keep getting this error -
Failed to execute query associated to this form.
After a form is submitted - I have loaded all files where they are supposed to be, can you help?
The form does get submitted and received, but this error pops?

Sounds like a mysql error. If your submitting to mysql did you make a database on your host?
I can't hear what I'm looking at.
It's easy to overlook something you're not looking for.

Did you create a database on your host? Don't create the table because the form builder will do that for you.

I had this problem, getting error message
"Your form could not be submitted for the following reason(s):
Failed to execute query associated to this form."
after the form was filled out and submitted.
I realized the database user was set-up as read only, changed to a user who could write to the database...and it worked.
Just my 2 cents.
