Hello
Is there a way for only one checkbox can be "checked". As it stands now multiple ones can be.
Is there a way that one radio button is not always checked? so that if there are 5 options they all show blank till one is checked?

If you want only one item to be checked, you should use radio buttons. To have a radio button not selected, check the box for it to be required.
