I've seen this type of issue after messing around with the php.ini. Took a while for the server to read it and do the update.

So, if I just keep uploading over and over, eventually it will go "what you doing ini here?' and let my form on in
Just got home from work – glad I can move on to the next problem with this site!

Been there done that. I was actually messing with the default time zone for a particular website. It took me two hours to figure it out, after writing a bunch of time scripts, that the server hadn't read the new ini file. Walked away and came back later and it was working. Obviously it wasn't as important to the server as it was to me.

Lazy servers!
That's good to know, I'm sure this will happen again.
