I need to create a form for the purpose of offering official documents to employees, such as an employee handbook, and have them verify they have read the document. Obviously we can't MAKE them read it, but, I want to offer the link to the document(s) (there will be more than one), and then have them select a check box to verify that they have read the document.

I have a form that I am testing, and I have a link to a document using an HTML element followed by a check box element with 1 check box. I would like for the check box element to appear once that link in the HTML element has been selected. I tried using a display rule so that the check box would only display if the link is selected, but I can only select from form elements except for the HTML element, which isn't offered.

Does anyone know of a way to accomplish this task? Is there an alternative to this that anyone has used?
