When I fill out the form and hit the submit button it generates a 404 error. I've tried changing the confirm options to all but the redirect to another URL but it makes no difference. Anbody have any ideas?

It looks to me as if the php files for your form are not in the directory of your web site where you thought they were.
ie. your form is here http://www.drdonnaparker.com/contact_form/contact_form.html
when I select the 'Submit' button, it is trying to go here http://www.drdonnaparker.com/parker_form_exported/parker_form.php
and it is not finding the files.
Your php files and form files should all be in the directory where you are displaying the form.
ie. http://www.drdonnaparker.com/contact_form/contact_form.html
You should have a php form file and 2 directories, a 'cartapp' directory and a directory of the same name as your form php file. All of these should be in the same directory. The one you are calling the form from.
Hope all that makes some sense. Maybe someone else can explain it better

Ok - my brain is like a steel seive sometimes. I should have remembered not to put the files inside of a folder. Moved everything to the root and now it seems to be working. Thanks all for the quick replies. Maybe next time someone can just slap me with a dead fish!
