Scott's technical support lead me in the right direction: an alert message "Internet Explorer restricted this webpage from running scripts or ActiveX controls." was appearing at the bottom of the page. I hadn't noticed it because my window was so tall and because the message automatically disappears after a few seconds. Another clue is that if the tutorial is published to a real website, then the pictures appear instead of the ALT text.
The problem is that the default IE11 security settings inhibit active content from running on the local computer. That setting defeats our ability to accurately preview a site locally and see the same rendition as would be provided on a real web server.
The permanent solution I used is from Dell's support site: https://support.software.dell.com/desktop-authority/kb/119158
1. Launch Internet Explorer
2. Navigate to Tools/Internet Options
3. Click on the Advanced tab
4. Scroll down to the Security section and Enable the option entitled “Allow active content to run in files on My Computer”.

The IE11 security panel says that the changes will only take affect after you reboot your computer. On my Windows 8.1, after I shut down all IE11 browsers and launched a new one, the new security setting took affect even though I didn't do a shutdown.
Since IE11's default security settings interferes with RSD's Preview capability, I suggest that CoffeeCup add the above steps as recommendations during RSD installation.
