What is the easiest was to add a contact us form page.
I already have a php page from my old site with a contact us form that I could use. Do I just create a static(?) page and copy the html (php) code to that page? all the code including the headers?

The easiest way is to just use the HTML Tool and paste in the Forum Builder code on the page you want the form to appear. Then just upload all the files to the root folder of your server.
