5. What specific steps have you taken to create this issue?
First, I've been using CoffeeCup for many years so am fairly well familiar with it. Unfortunately, I can't tell you how long ago this started but now, when I want to work on a page, I double click on the page I want to pull up in the editor and the page will blink once or twice and then the tab at the bottom displays an asterisk as though a change has been made on the page. I can't figure out what change was made where. Sometimes it seems that the program will add a "?" at the very start of the page just prior to the <html> tag.
6. Does it occur every time? No and I'm not sure why. I can't figure out if it is the specific coding in each page, like the existence of some java script on certain pages.
7. Can you recreate the issue on another computer?
Yes. It happens on my Windows 7 Professional, 64 bit machine as well.
It's only certain pages.
The index.html page does not do this when I open it.

It could be your anti-virus/anit-malware program. Try disconnecting from the internet turning off or disabling your securtiy software and then check to see if the problem persists. Most security software does a scan when a file is opened or data is loaded into memory to check for malicious code. It could be scanning the file slower than HTML Editor is loading the file.
I once had a server doing the the same thing with a particular external style sheet whenever I'd upload it via ftp. Changing security software remedied the problem.
Don't know if that is your problem but the prefixing of the file with a ? is the same file change I experienced.
I think it is a little funny that the back button doesn't reckognize a change but the Editor reckognizes a file change. I wonder if you changed the filename if the issue can be recreated?

I'm thinking it's something with the editor myself too, or something with the code we are using in the editor. Something added that is confusing it (or in my case was in the default theme I chose which wasn't a CC theme) that makes that question mark show up in the front.
I had a ticket a couple months ago with this same issue, and never did resolve what caused it. It didn't "keep" happening at each opening of a page though, only the first time, then once I removed that it was fine so I dunno what the deal is with that either, just confirming that you aren't alone on that issue.
