Problem with extended replace.

Running this as administrator, when I do an extended replace it either works on the one file I have open, or just a few files, basically I can't trust the results. Urgent problem as I have 40,000 plus pages to do this on!
It used to work!

All my settings are ok, "include subfolders" etc; etc. Many many years experience of using editors (dreamweaver, homesite, komodo, matrix etc so I don't think I'm doing anything dumb here.

Please advise.
