We have a proxy filter that pops up when a user access a site that is blocked for whatever reason. What I need is a form that will capture their username $(user.name) the address that was blocked $(url), the category that it fell under $(category) and a textbox that will allow them to add a reason why they feel it should be unblocked. Once the user completes it, it will send it to an email address that I define. Is that possible to do in CoffeeCup HTML editor or do I need additional software?

I already answered this question in the FB area but I'll answer it here, as well. If you know how to code the php to capture the information and insert it into a form, yes it can.
