Doug, much as I hate to say this, I think you are still not getting ALL the files uploaded. Or all the old ones deleted. The Web form Builder does work great, and is fairly easy if you hit all the steps.
I would suggest taking a deep breath, then deleting all the form builder files from your server.
Then go into VSD and select Edit/ Remove Files and make sure ALL files having to do with the form are deleted.
As far as CAPTCHA, there is a work-around. Simply add a 'required' checkbox that asks 'Are you human?'. Users will have to check the box to submit the form. Might not be perfect, but does cut down on a lot of the spam.
Once you have your form the way you want it, re-copy the code to the HTML box and place it where you want on your web site. Then publish your web site.
Go to the directory where you saved your form on your computer. There will be a directory yourformname_exported
Change to that _exported directory and you will see files and folders. Use DirectFTP to send all these to your server. They should be in the ROOT of your web site.
At that point, if all went well, your form should display.
Things to remember...don't use any 'spaces' in file/folder names. Make sure your HTML box is the same size your form will be and doesn't overlap anything else.
Let us know how it all goes. If it doesn't work for some reason, leave all the files you uploaded on your server, and post a link to your site with the form. There are those here that can look at it and tell you what went wrong. Good luck..
