How do I get this to put a form into my pages

You create the form and it will display the code you paste into your web page. You then upload the page and all the associated form files it tells you to upload to your server.

Yes, but where do I save the form files if I want to integrate them with the Designer ? In the designer folder, or in a totally separate folder ?

If you don't want to modify the html code FB produces, put everything in the same folder as the site. Otherwise, you will have to edit the path in the html to include the folder.

My plan is to insert the code created by the FormBuilder, into one of the pages created by the Designer. And I'm not planning to modify that code.
So based on that:
If I put everything in the same folder as the site (as you recommend), should I let the Designer program upload the site that now includes the form and the Form files to my server ? Or shoud I let the Form program to FTP (upload) the form files to my server ?
The folder where the Designer program is saving the HTML files in my C: drive is:
C:\CoffeeCup DERMCONSULTATION.COM\Derm1-v6_website
So the Form files should be in the same folder shown above, correct ? (Where the HTML files are). I guess that when I save the form files I will have to switch to the Designer, insert in my page the code that I copied from the from, and then tell the Designer to upload the whole site.

If you use the flash tool (providing we're talking about Visual Site Designer) the files will be automatically be added to your website folder and at that point you'll be able to preview it. If you wish to save the form files somewhere for editing later, what I did was make a folder in My Documents named Website Forms and saved all my form files there.

Fantastic explanation. So I will run the Form Builder from within the Visual Site Designer, and the Designer program will take care of the location of my Form files.
