Does VSD have a feature to make a fixed width page on top of a larger background? I see sites like this all over the web. EXAMPLE: http://www.comcast.net/

Is the trick to build a separate page and the import it into a wider one?

*Some VSD templates achieve this look by using large objects as palettes. This makes the page cumbersome and slow to load.

Yes, you can do that in VSD. When you start a new page, you select the page background (colour, image or gradient). Then, with the page open in the programme go to Settings - Background fill properties. You have three options. The first one continues the chosen background beyond the limits of the container, the other two set a different colour of your choice or an image to continue outside the limits.
