I created a form in CC Form Builder, inserted the code via insert HTML in VSD, uploaded the form files to the server, but this is how it's displaying.

I've done this before without a hitch, but I can't think where I'm blowing it. Help!

A few things:

1. You are using the Classic version of Form Builder which has been discontinued. Just saying.

2. You are 7.0 build 8 of VSD which is considerably old. Build 26 is the most current.

3. The Flash file is missing:
