Web Form Issues

I have created a form as an "Application" for customers to fill out and then send to me.
I have been getting errors and when the "submit button" is clicked on, an error message page pops up.

I have un installed and reinstalled the web form builder software program. I have changed the "permission settings" on each file and on the page where the form sits.

My question is do I need a dedicated Ip in order for this to work? and or Do I need an "SSL Certificate"

I have also tried changing the email to one I can use directly on my web host provider, but still not working properly.

In the email settings how do I know if I have a"linux" pr "Windows" server...I have had the Linux one checked.
I have checked and all of my files have been uploaded correctly associated with the form created...Im lost..

the form is on the "Application" page...when you click on submit you will get the error message.
