after i complete the form fields, when i click on the submit button it doesen't show nothing , earlier the form was work , and after i add a few input boxes and try again it doesen't works
why?

i upload all files,
the form is not send me a email with registration details, earlier when i click on the submit button appears a message with something like that:
"thank you for your submision .... "
now is not work, it blocks the entire form i can't make nothin i need to refresh the webpage
