Importing CSV file including "Real...

I have no problems importing just the username and password, works fine everytime.
But WAM has a very nice feature of recording the users real name, and in fact fills the "real name" field with a copy of the username.
Because the username can't accept duplicates, we use a unique number in that field and the users surname as the password.
Question: How do I set up the CSV file to import realnames, usernames and passwords to the correct fields?
I think I've tried all the different ways of ordering the CSV file, but with no luck so far.

Can the USERNAME field accept duplicates, with each having a unique PASSWORD?

The USERNAME field must always be a unique name. If you think about it, how would the system know which password to use when you tried to login if you had two usersnames the same.
