I am trying to put a form on a Wordpress page but when I update the page the code disappears.

I have uploaded the other files created to the server

One issue could be the spaces in your name. It is never a good idea to have spaces in files on the web. I think you sound re-save your form from Customer Enquiry Form Dial A Dog Carer to customer_enquiry_form_a_dog_carer and see if that helps.
