created form, not pop up,
placed resulting code in HTML element in RSD 1.5
preview mode shows no form in any browser.

I think I followed the tutorial I found correctly(several attempts)
as a test I also tried to use it as a pop up and pop up showed partial form in safari, OK in chrome and no-show in firefox

Your form won't show up until you upload it to the web. I keep a test domain for testing things.

Try uploading your page with the form and you should see it that way.
