If your referring to the provided script, it has to do with where you publish your form files compared to where the page is located that contains the script.
I can't hear what I'm looking at.
It's easy to overlook something you're not looking for.

So, but both the form files and folders and the page on which I'm publishing the form are on the same place. The form files are on the public_html folder, the same folder on which I have my WordPress installation. And, I did embed the form on a WordPress page.

If your using the script as is then the page your form is on needs to be in the same directory as the myformname.php and the myformname folder. I usually keep all the files together in the file that the form builder creates. myformname_exported is what you'll see and I usually remove the _exported from the file name and adjust the script to point to this folder by adding myformname/ to the scripts url reference.
