Take your time and look at your input code to make sure everything is there.

I actually figured that on out on mine, very proud lol.
The form is displaying perfectly and showing the values in the Trailer Input Boxes.
slight issue with Cumulative total but probably a spelling mistake on my part.

Main concern is after pressing the submit button it wants to go to:
https://files.farmtronics.com.au/004WZ_ … /004WZ.php
And nothing Displays and eventually i get it time out error.
Shouldn't it be going back to:
https://files.farmtronics.com.au/004WZ_ … /004WZ.php

Not sure why it's hanging there but what I would do is go to the Form Builder and in the Settings/Confirmation setup your form to use a custom redirect option. In that enter your link.

You'll need to re export and add all of the custom code back to the form and test.
Do you want a confirmation page?

Really,
I tried on my laptop and iPhone and it hangs.

Don't require a confirmation page just need the form to reload ready for the next entry.
Normally just use the Go Back Button on the standard Form Submission Success page that comes up.
Might do a restart on the old PC.

Can you zip up your actual directory for your test form so I can test it here? I don't want the fb file I want the entire directory that you manually exported.

So it seems like a browser issue.
I saw the same thing with your form that you reported.
I saved your form as 004WZ.html and uploaded the form and submitted it. It was showing the php code within the inputs because of the page now being a html page instead of a php page but it submitted and showed the confirmation. I hit the back button and it took me back to the original 004WZ.php form and I then submitted that and it worked. Very strange behavior. I'll do a little digging to see what could cause this.
