new form not showing up

This form works, it was one I made a while ago.

This one does not, it looks like the new one may be responsive and has php script with it.

Not sure if you need that info but thats what is on my server.

Hi Logan,
Remove the spaces from the form name.:) It's always a good idea to keep spaces out of code. Use under scores or dashes if you want some separation for readability.:)
I can't hear what I'm looking at.
It's easy to overlook something you're not looking for.

Got it to show up but now it wont submit. It gives the following error.

Your form could not be submitted for the following reason(s):
Failed to open database [2013] SQLSTATE[HY000] [2013] Lost connection to MySQL server at 'reading initial communication packet', system error: 111.

That file is there, I can access it through the file manager on my server. I can even click test on it and it says, cool, sessions are usable.

Thanks for the help, I made a different form without the responsive feature for now and I got it working.

I don't think the form be responsive had anything to do with it. I think a few files didn't make it to the server.;) Your form is still responsive for me.:)
