I have created a form for my website, and uploaded it manually to my server. When I pull up my site, click on "enroll" and fill out the form, I get this message:

Your form could not be submitted for the following reason(s):
Warning: [ err 2/line 166/datasavecsv.cls.php] rename(/home4/kartsqu1/public_html/onlinearted.com/enrollment_form/storage/csv/form-results.csv,/home4/kartsqu1/public_html/onlinearted1.com/enrollment_form/storage/csv/form-results1.csv) [<a href='function.rename'>function.rename</a>]: No such file or directory
Failed to archive data file.
Failed to record the data because the server is too busy or doesn't have write permission.
Failed to send an email to "amy@onlinearted.com".

Can you tell me what I am doing wrong?

Looks like you are logging to CSV. What you have to do is with an FTP client, connect to your server and change the permissions of the storage folder to 777 for that folder and all under it. This will then fix the problem.

I tried that and got the same thing. I went through my cc ftp and found the enrollment form folder on my site. I changed the permissions to 777, although the 0 would not delete, so it is 0777. I applied it to all folders. Could I have done something else? Maybe I should delete the enrollment form files off my server and try resave everything?

Make sure there's a storage folder on the server. I had to ftp an empty one up to the server to get one of my forms to work.

I could not find a storage folder on the server. Should I create one and place it in the folder where my site is, public_html? I have the public_html folder, and a few sites in that folder.

You should see a directory structure like this.
myformname/storage/csv/

Okay. I found that. Do I create a folder called "storage" in the csv folder?

Then set the permissions per Scotts post.

Okay, I did that, and this is the new message I get:

Your form could not be submitted for the following reason(s):
•Failed to send an email to "amyspoint@aol.com".

So at least it's a shorter failure message

Do you have that actual email address?
