multi-column grid rating scale?
A client has asked me to produce a form that has an element comprised of a multi-column grid of ratings. I've attached an example image of what they're needing. Is there a way to do this with Web Form Builder?
Thanks for your help!

Hi Brad,
Do you want the form to be responsive or are you looking to have a fixed width form?

I can't hear what I'm looking at.

Hi Eric,
Responsive is always ideal,
, but certainly not a deal breaker! Before I set out and dig through CSS, I'm curious if there's a way to approach this in WFB's excellent GUI. Thanks for your advice!
Responsive is always ideal,

Within the app itself there's not a straight way to do this and get that exact look. The form builder has a min of 20% width for elements so that leaves you only 5 elements side by side. You could use an html element to get that exact look then target the inputs with jquery to change values of inputs hidden in your form.
