Issues with IOS Mobile Devices

I have the latest version of Form Builder registered version
When a player comes to this page, they can select a registration form under the corresponding category for the team they wish to join. Using an Iphone (IOS based) device they are able to enter all of the information without any issues. When they click the submit button, the page simply refreshes and takes them back to the blank form.
The forms are formatted to take them to a payment link. The forms work without issue on Windows devices. They also work without issue on Android devices. The issue only happens with an IOS based mobile device.
We have tested Iphones and ipads, with Safari, Chrome and Firefox with the same results.
I am extremely frustrated with this issue.
I tried contacting coffeecup support and they were no help and sent me here.

I can't hear what I'm looking at.
It's easy to overlook something you're not looking for.

Eric, Thank You for finally getting to me. I will say this.
I cant fix 56 errors because I have no idea what they mean. I dont want to sound stereotypical but I know mechanical things. I can build guns in my sleep, program and run 3d prototypers and cnc machines. I can not, however code a website.
We use sitebuilder.com for this site. Everything is in a prebuilt GUI that we use because I dont know a thing in the world about HTML. We have used Coffeecup for the better part of ten years because it is is easy to use and doesn't require coding.
None of this explains to us why it is that these forms work on every platform we use except IOS mobile devices.
Windows works flawlessly with Chrome, IE and Firefox.
Android devices work flawlessly.
I am sorry but this is driving me crazy and so far nothing has worked. I am a dumb country boy. I get it, but I am not an idiot.

Try this Thomas. Instead of testing your form that is embedded into your page, go to the form directly and then test it on your iPhone and see what that does. This would bypass all those errors.
