Once again, Your form here will not let me upload the .fb file extension.
If I open up that file, it just loads up the form builder.
How am I suppose to send you a fb. file?

NO I DO NOT KNOW.

Finally! :)

Fixed a few things:
1. You still had the file upload size set to 100k. Updated that to 8102.
2. You did not set a subject line. I set that to You just got an message from [email3].
3. You did not define a From address in the Notifications.

Tested this out on our servers and the file upload worked and the notification email looked great. Remember you still MUST fix the permission issues on your server.

Now it says my username & password are not in the system, so I cant publish.

The form you sent had your email username on it, so I tried to put my email and password in, then tried to publish, and said no record of that username or password.

Refresh my memory on what permission issue on my server.
I will relay the info to my data center.
