Sorry it's me again Eric, I forgot to ask but how would this work if I had another question that used radio buttons within the same form? I'm guessing this would have to be defined in the script?

Yes the script would need to be adjusted for any radios that use the same 1-5 values. I'll need to see the finished form in order to make adjustments.

Since adding more questions with radio button options these are now conflicting with others, help... please
I noticed the script had not changed to the original, is it possible to make it specific to that one question?

The script will interact with any radio buttons with the same value of 1-5.
