Tim, What Scott is asking is what is the web site link for your form such as:
www.yourdomain.com/form.html

That way, he can try out your form and see what the problem might be.

Yes I figured out why you asked for the URL. I didn't quite do the server test quite right

These are the results....

Server configuration

FormBuilder version: Web Form Builder (Windows), build 2.5.5437

PHP Version: 5.3.3 (should be 5.3 or newer)

Curl extensions: OK

Mail configuration in php.ini file:
Sendmail Path:/usr/sbin/sendmail -t -i
Sendmail From:
SMTP:localhost
SMTP Port:25

Data base access:
MySQL: Installed
SQLite: Not Installed
PDO - MySQL driver: Installed
PDO - SQLite driver: Installed

File upload configuration:
File Uploads: On
File Uploads Max Size: 200M
Post Max Size: 200M

PHP Sessions:
Cool, sessions are usable!

It says it ok so I'm guessing I've uploaded the files correctly and the servers ok. So what next.
I'm going to test this as well....
Note: Some hosting providers will only process notification emails if you're using an email address they provide. If you're unsure of what this email address is.....

I tried to test your form Tim, but you don't give any indicators on the phone format (is it mobile, US, UK, etc.), so I don't know what numbers I should be using to make an submission. Make use of the Friendly Hints or Error Messages so that you can display alerts when people try to use your form but have entered incorrect data.

If you look at the attached file Notification 1 thats how its set up now and comes up with an error
Ive tried "YOUR FORM COULD NOT BE SUBMITTED FOR THE FOLLOWING REASON(S)" method with inserting another email as in attached Notification 2&3. so i guess this is pointing at my server not handling the form unless they host the email account (im using Google apps).
I did find this on their website but im not sure I understand it fully!!

You will need to contact your hosting provider and ask them to explain why the form is not sending emails correctly with PHP. Thy might have other restrictions not documented.
