Scott - thanks for the link - that makes me feel better that at least I uploaded it!!

I filled out the form though (as a customer would eventually) and it appears as though it won't allow me to submit it because of an error. I'm guessing that's due to me filling out something wrong within the settings of the form ?? Attached is the error I'm getting

I'll fill out a help desk ticket if you guys still think I should - amazing response time by the way - wow

WHOA!! Eric you're the man. It worked!!! Thank you So much... you have no idea how frustrating that was - my wife will be thankful as well as I have been an absolute grump.

My next hurdle is actually being able to fill the form out and figuring out how to access the stored .csv file.... any clue where I should go from here ?

I appreciate the time you've both invested up to this point - is there a help document somewhere that I can reference on how to troubleshoot why my form won't fill out...... and/or how to access the .csv file ? I hate to waste any more of your time if something exists out there already....

The issue there is with the MySQL information. What you have placed in the setup is not correct. For Godaddy you want to make sure you copy and paste the exact same information they give you in their control panel when you created the database. I would actually recommend you create a new database just form Form Builder so it is separate from Wordpress. Godaddy is VERY picky on the MySQL settings for security reasons.

To access the CSV file, FTP into your server and go into the WebForm_1/storage folder and here you will locate the CSV file.
