I have a site residing on a company test bed

The form is set up to go through to an info@ email and I know that that email is working. But the form responses do not go through. I get a response after submitting on the page but nothing gets to the person.

When your dealing with a script executing on an intranet you will need to get with the IT people to make sure they have everything in place to allow that. SMTP mail, SESSION's and the proper PHP version are all needed.
On the host goto myformname/fbapp/php/servertest.php and see what it says.
