Form format changes when published

What does it look like when you preview it? Maybe reexport it with formbuilder then publish again.

The problem here is with the form files. It appears all the markup was somehow changed. Go to http://174.120.23.187/~botegga2/linda/r … quest.html and view the source. There are references to the _exported folder. The CSS file points to this location:

Form Builder never adds that markup, so I am not sure how that got into there. Are you adding these files into Drewamweaver and uploading through there?

You can use Dreamweaver. What probably happened is you moved the files around within the project. When you did that, it automatically updated all the file references within those scripts. The next time you do that, click NO when Dreamweaver asks if you if you want to update them.
You will need to delete the form from the project and re-add it. Also, you are using a much older version of Form Builder, so take this time to also update to the latest version.

I have answered my own question. although, i believe that i have loaded the files into Dreamweaver before uploading in previous websites.
