How can I change this in FB?
When I get an email notification, the subject line has this:
<table><tbody style="vertical-align: text-top;line-height: 30px;"><tr><td>Your Name::</td><td>Jeff D</td>..............
It's also displayed across the top of the email message.
I didn't see anything in the settings for a 'subject'.

This is a form published to SDrive if that makes any difference.

Sounds like you have your email address setup wrong in the form builder. Check for an incorrect character or extra or missing one.

OK. I had this happen on the last 2 forms I made as well (different emails). Better get some new glasses.
