Yeah, sessions are enabled so the problem is with permissions. I would disable MySQL, CSV logging and CAPTCHA and then try again and see if that helps.

Captcha I can turn off, but the logging isn't turned on at all to start with. Captcha being off permanently though isn't an option so hopefully that's not the problem.
Removing Captcha didn't do anything, error still there. I also checked just to be sure again (which I had already done earlier) that there are no setups for saving the data anywhere. There is nothing checked for that at all.
So nothing so far is getting rid of this error. The forms both seem to work fine, but the one gets the error. I am thinking of redoing it from scratch since a pretty simple little form, but the client is asking what the questions would be to ask the hosting company and I've no idea at this point. The forms are working, just not for him. The error was not there before which leads me to believe it's something they did server side, although I have no idea what that could be.
The client is not getting the emails from either one of his sites and he was definitely getting them before this new site went live and before the hosting guys transferred and screwed up royally his email servers. They fixed that I think, but now I'm not so sure lol, maybe it wasn't fixed fully after all and we just didn't know it. I'm at a loss here, I'm not a php person and I really don't think it's anything I can change, but I have no idea what type of questions he needs to ask his server guys. I don't know where errors like that are generated from. Soo.....

I would try a new form and test but if that fails, probably have to talk with the provider and see if they have any log files they can look at to determine what the permission issues are.

Well damn, new form did not fix it. Totally redid the form from scratch. All files on server were deleted completely so no overwrites or residuals left over. Same error 
I have sent the client an email because at this point I'm thinking that whatever happened to cause his domain account emails to stop receiving the form, is also what is creating this error. Is that a possibility? Almost has to be as this is way too coincidental that both things would happen at exactly the same time. Adam will most likely remember what a pain it was to work with this hosting company years ago, it's no different now. I thought it was better, but I am rescinding that thought as of today lol. I am hoping he decides to change hosts as he talked about it this morning, but we'll see.

have you tried removing all the .htaccess files the formbuilder places?
I can't hear what I'm looking at.

Am back home again, sorry took so long to answer. I can see no htaccess files anywhere that the form builder would have added. I have hidden files shown in my ftp program, and there isn't any anywhere. I went through every directory of the form files and nothing. Checked to make sure they were visible by checking my own domain and I can see them in there so... nope no htaccess files exist at this time.
I have written a long and drawn out letter to the hosting company in hopes that with having all the information they can look into this further.
But .. I just read this thread and am wondering if maybe it might be something like this that is happening here?
