edit form email message?

When I get a form reply in my email, it shows this across the top:
<table><tbody style="vertical-align: text-top;line-height: 30px;"><tr><td>Your Name::</td><td>John Doe</td></tr><tr><td>Email::</td><td>johndoe@email.com</td></tr><tr><td>Phone #::</td><td>0001234567</td></tr><tr><td>Questions? Comments? .....

The form results/message show up as expected in the body of the email, so I don't need all the code across the top, but I can't find anything in the settings to get rid of it.

I can't hear what I'm looking at.

When I use my actual website email (me@mysite.com), it comes from "my-private-user-name@my hosting co name.com" instead :o And I get the form submission (as above) in HTML code.
When I use my other original Yahoo email address on the form I get the same thing.

I added the me@mysite.com to the Yahoo account several days ago, but am unable to login to confirm the change.
Tried it again a few minutes ago, now I'm locked out for too many attempts to log in.

Regardless of email address, I get the form results in HTML across the top of the email.

I don't know anything about this. I've got the latest version of FB, but don't see anything about a stock email option. Where would it send the form replies to?
I put my email address in the notification so the form results will come to me, which they do. I just want to figure out how to get rid of the displayed HTML code and keep my hosting user name from going out on form confirmations (instead of my email address). I guess I'll get rid of the form for now.
