How do I create the tables in my database? How do you access the stored data there? Is there an admin section where I can login and see the info when some on submit the form? and when using the my own server, will it work like the s-drive, like clickable links to the submitted from? Thank you.

To work with Mysql and form builder you setup the database on your host. Now you enter all the necessary info into form builder so it can login to your host to create the table that you name in form builder. Don't create the table with your host let the form builder do the grunt work when the form is submitted. Now you just have to create some php and mysql scripts that will extract the data then apply a little css and html to format the display of this data.
