Conflict with Forms?

When I add a contact form (created with CC FB to a page that has a Mailchimp newsletter signup form on it, the MChimp form loses its styling.They are in separate divs in different areas. Any idea what I can do to have both forms on the same page?

Also, I keep seeing this everytime I preview the page:

I even removed the contact form code/folders and it won't go away (Win 10, Firefox).

Is bear-contact the name of your cc form?
Can we see the page?;)
I can't hear what I'm looking at.
It's easy to overlook something you're not looking for.

Sorry, the form script, folders, everything, has been removed. Only that stupid pop-up remains.
I just put an email link in place of the form on there.

I probably need a stand-alone form page to keep it from conflicting with the Mail Chimp signup.

I'm thinking the best solution would be to pay someone else to create my forms. I just don't have enough time to mess with these things, and I tend to have issues with most forms I create. :P

I always get something like that if I try opening a local php file in a browser. It will open in the Editor preview, though.

I'm not trying to open it. Usually, in preview, there's a blank space where the form will be – and no pop-ups.
I don't get this in Win 7, just Win 10.
I'd like to find some way to turn that off so I can just preview the page without that pop-up on top of it.
