I need to change the values of CITY drop down depending on the STATE selected above.

Is it possible do it either by CSV input or MySQL ?

You can do this with the built in conditionals in the form builder.
Make a drop down with all the states in a list.
Now create separate drop downs for each state that have the cities you want. Now you can create the conditionals for the city list dependent on the state selected.
