Can we use a ranking images?
Hi! I'd like to use our form to get Testimonials or reviews from clients...directly to us, not through Yelp, etc. Is there a way to use "Stars" or even better, other small icons (5 dogs, 3 cats etc) in the form?

Sure. Add the images 1 star 2 stars etc.. next to a check boxes then when the user submits their vote you post it to a mysql and pull the data to a results page using a little php.
This should get you started.

One step ahead of you on that, Eric! Thanks for the advice. I've also suggested to Mr. Scott that it would be nice to see a ratings system of stars, thumbs up, or fingers up the nose

Here's what I came up with: a fun way of using Web Form Designer as a way to include a ratings system on a testimonial/review page I've done for one of my colleagues - a veterinarian. Here's the "test" form:

It works really well on default theme - if you want to change the form theme, you will have to simply retweak the font color to make sure it blends into the background and tweak the image padding to line up the radio buttons with the image lines of stars.
