Not sure what you are asking. The form you are referring to cannot be filed in on-screen, it has to be downloaded and either printed and filled in by hand, or downloaded and converted to to something editable, for filling it in still on screen.

Forms built with CC form builder can be filled in online and submitted.

Use CC Form Builder or you can use Adobe Acrobat Pro 2017.

The kind of form you show as an example would be easiest to create (first in Word) then saved as a PDF, opened in Acrobat Pro to finish the details. You can complete them on-line and then print them out for mailing or add a "submit" button and have the form sent to an email address or url.
