1) What is the "normal" process when people change their forms after data has already started getting captured? i.e. add new fields
2) Is is the "normal" process that when you upload the new form, it doesn't actually archive the old table and create the new table until you physically enter a record through your form?
3) Anybody know how I can create a SQL file of the data in my CSV file?

Using Linux hosting with cPanel, MySQL and myPhpAdmin. Created a database for my form results. Everything was going well until my user requested some additional information be captured so I needed to change the form and have the table re-created (and the original one renamed, per CC user guide) and then copy my data from the archived table to the new table.
1) Because I was nervous about doing this for the first time, I made sure I exported all my existing form data to a CSV file on my client. I chose CSV because mnPhpAdmin only allows for imports from CSV or SQL tables.
2) Changed my form, saved it, and uploaded it.
3) Went into myPhpAdmin on my hosting server and the new fields were not there. After trying a few things and re-uploading over and over, I finally deleted the entire database and started over (still have my CSV file on my client so I'm feeling ok about doing this)
4) Table still didn't create after uploading form so that's when I finally went and entered a dummy record and voila, the table was finally re-created.
5) Using myPhpAdmin again, I went to upload my data into the table using the CSV with Data Load method. Set options to specify how fields were ended in the CSV file (by a comma), etc and uploaded it.
6) Program said it uploaded, but when I would browse my table, every record had an id number, a date/time stamp of all zeros, and all the remaining fields were blank.
7) Tried setting the CSV with Data Upload options a little differently, but got the same results.
8) Contacted web hosting service and they told me to create a SQL file of the data, upload that, and they'd take care of the rest.

Problem is, I don't have a clue how to create a SQL table. I've been going back and forth with them for 2 days now...there is a language barrier and they just keep telling me to create a SQL table. I keep saying I don't know how and now they've asked me to call back in 7 hours when their 2nd level support will be there.
