It's been a long time since we heard anything from CC about the Form Designer. I only hope that means they are working on it to make it a full-fledged form creator, with front end, back end and everything in between.
I have a form in my sharing folder, including a back-end script. It is a simple form, so if your needs are not too complex, take a look and see if you can use it. There is a version for RSD (or RFF and RBB), and also for SD3, Bootstrap and Foundation versions, made with those programmes respectively. URL in my singature.
